Job Description 

  • Spinal Cord Injury Registered Nurse
  • No of Contractors required: 4
  • Work site: Department of Veterans Affairs Clement J. Zablocki VA Medical Center 5000 W. National Avenue Milwaukee WI 53295
  • Period Of Performance: The period of performance will be a 12 months base from the date of award with 4 consecutive one-year options.
  • Service Contract Act: No
  • Requested Bill Rates for Supplier: Max of $71/hour
  • Due date of submission as of now: 14-June-2024 2PM EST
  • AUTHORITY: Under the authority of 38 U.S.C. 513, the contractor agrees to provide Spinal Cord Injury Registered Nurse Services in accordance with the terms and conditions stated herein, to furnish to and at the Clement J. Zablocki VA Medical Center (Milwaukee VAMC) located at 5000 W. National Ave., Milwaukee, WI 53295.
  • In accordance with FAR Part 16.505(b)(1) Task or Delivery order placement procedure, the Great Lakes Acquisition Center will provide all awardees a fair opportunity to be considered for all awards under the VHA Integrated Critical Staffing Program (ICSP) IDIQ contract.
  • 1. SERVICES
  • a. The contractor(s) shall provide the services of up to four (4) Spinal Cord Injury Registered Nurse in accordance with the specifications, terms and conditions listed herein. All work performed under this task order will be completed at the Milwaukee VAMC, Spinal Cord Injury (SCI) Service.
  • b. The period of performance shall be for a one (1) year task order with four (4) option years to extend services. Contractor is to provide firm-fixed pricing in the Price Schedule. The quantities in the Price Schedule are estimates only. Contractor will be paid only for actual services performed.
  • c. As listed below; the contractor shall submit with its offer the following documents:
  • • Curriculum Vitae (CV)/Resume of the proposed service providers
  • • Copy of all current licenses/certifications of proposed service providers and competency checklists
  • • Three work related references for the providers
  • • Information on malpractice claims, termination or suspension of privileges, or information on any occurrences of removal from an assignment, as applicable.

  • d. All offers must be submitted by June 17, 2024 at Noon, Central Time.
  • Award of task order(s) will be made to the offeror(s) whose offer represents the best value to the Government based on technical capability, candidate qualifications, candidate availability, past performance, price and other factors considered.
